- Referral for further evaluation or treatment
Regular participant monitoring appointments with CPHP
Tissue Testing/Urine Drug Screen monitoring
- Case Management Services
- Reports generated to various entities
Useful points pertaining to Treatment Referral & Monitoring:
- CPHP does NOT provide treatment. We do provide treatment referrals, as we have an elite cadre of highly qualified professionals in the community who specialize in working with medical professionals.
- CPHP also refers to specialized national programs if a condition requires treatment not available locally.
CPHP will monitor an individual for the duration applicable to conditions/situations presented by participant.
- CPHP may collect on-going collateral data gathered from treatment providers, family members and/or workplaces, if the CPHP client approves the requisite release to do so within CPHP’s confidentiality guidelines.
